Participation Agreement, Waiver & Release

  1. Assumption of Risk: I understand that participation in activities including Laser Tag, Arcade Games, Bowling, Kids Playground, and event areas involves inherent risks such as slips, falls, collisions, equipment-related injuries, low lighting effects, and physical exertion. I voluntarily assume all risks, including those arising from negligence.
  2. Release of Liability: I release and discharge Blaster Powerhouse, 2569921 Ontario Inc., its owners, management, staff, officers, employees, contractors, and affiliates from any claims, damages, injuries, or liabilities arising from participation, including those resulting from negligence.
  3. Indemnification: I agree to indemnify and hold harmless the facility from any claims brought by or on behalf of myself or any minor participant.
  4. Minor Participants: By submitting this form, I confirm that I am the legal parent or guardian for each child added to this family profile.
  5. Medical Consent: I authorize Blaster Powerhouse staff to obtain emergency medical assistance if necessary and accept responsibility for any related medical expenses.
  6. Governing Law: This Agreement is governed by the laws of the Province of Ontario. Any legal proceedings shall be brought in Ontario courts.
